fix(mssql): clean up module and add runtime debug logging control

- Fix indentation and lint issues
- Correct message normalization and ZNC callback handling
- Prevent private action logging from dereferencing missing users
- Add Debug on/off/status module commands
- Disable debug.log by default while keeping error.log enabled
- Include debug state in Status output
